This Accreditation must be reaccredited at the end of the accreditation period. This Certificate and the marks “BHCOE” and “Accreditation” may only be used in accordance with the rules and standards of the BHCOE. This Accreditation is a limited license to use the BHCOE accreditation mark listed above, subject to continued compliance with BHCOE Standards. This Accreditation may be revoked or limited in accordance with the BHCOE rules. This Certificate of Accreditation must be returned immediately upon request by the BHCOE.
